In addition to submitting the FSU Application for Admission and the College of Music Supplemental Application, applicants should also submit the following forms:

• Letters of Recommendation
  • Undergraduate Applicants: please use this form.
  • Graduate Applicants: Although letters can be mailed to the College of Music using our form, we strongly recommend using the online process conatined within FSU's application for faster service.
• Repertoire List
  • Please use this form as an example, create your own, and upload it as part of the College of Music Application.
• Supplemental Music Theatre Application - Music Theatre applicants should plan to bring this completed form to submit during audition check-in.
• If you are a transfer student and receiving scholarship, you must submit a Transfer Release form
