The audition deadline for Piano Institute and Honors Chamber Winds has now expired. (These are the only two camps which required an audition for acceptance. All other camps you can just register!)

At this time, we have awarded all of the EVENT (ie. ALL STATE/TRI-STATE) and WORKSHOP Discounts that we have funding for. However, if you had plans to send in applications for one of these discounts, we encourage you to still send in your applications. Any Event or Workshop Discount that is postmarked on 3/27 or after will be held in a queue. In the event that we are able to give more discounts as we get closer to camp we will certainly do so, and they will be processed in the order they were received.
REQUIRED CAMP FORMS PACKET:
**This packet should only be filled out by campers who are REGISTERED for camp. All registered campers will receive this packet of forms in the mail. If you are not sure if you have registered for camp, please call us to verify.**
To help us prepare for your arrival, we must ask you to fill out some information forms. Some of the forms are required for camp participation, and some are for optional services. Parents should read each form carefully and follow any instructions. Some of the forms will require you to repeat information, and we understand that this can be tedious, but many of these forms go to separate offices. For accuracy and for your child’s safety, we ask you to please fill out each form in full.
Please complete the forms and mail them back to us as soon as possible. The deadline to submit them is no later than two weeks before the start of your camp, but the sooner we receive them the better. We also suggest making a copy of the forms for your records and bringing them with you to check-in in case there are any questions or problems.

CAMPER SIGN OUT / PERMISSION FORM
(This form will also be sent to you in your Packet of Required forms).
Use this form to give permission for your son/daughter to be picked up or signed out from camps by someone other then yourself. It can be returned to us in person, by snail mail, by fax or by e-mail.
PARKING PERMIT REQUEST FORM
Day-campers can use this form to purchase a parking permit if they are driving themselves to and from camp each day. This form can only be returned by mail along with payment for the permit. Deadline is two weeks prior to the start of your camp.
Housing Requests Forms
(This form will also be sent to you in your Packet of Required forms).
Use this form to request a roommate or to order extra nights of housing in the dorm.
CD/Photo Order Form
(This form will also be sent to you in your Packet of Required forms).
Use this form to order a CD and/or Photo from your camp.
